Your First Visit
Dr. Diana Ponoran and staff are passionate about providing you with the highest quality care and results that you’re proud to show off. Our administrative staff is ready to help you with any questions about scheduling, financial policy and insurance; we’ll make that part of the process as simple as possible.
This section offers information from the administrative side of Angelique Dentaire, including what to expect on your first visit and what you should bring with you.
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Your visit may also include a review of your medical and dental history, a dental examination and, when appropriate, a professional cleaning performed by our dental hygienist. Occasionally, treatment can be completed on the same day as the consultation.
During your first visit, our team will take the time to discuss your questions, concerns and goals, and create a dental care plan tailored to your individual needs.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any X-rays taken within the past 6 months, if available
- A complete list of medications you are currently taking
- If you have medical or dental insurance, the necessary completed forms and your current insurance details
This will help us process any insurance claims in a timely manner.
IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.
X-Rays
If your previous dentist has taken recent x-rays (within the past 6 months), you may request that they forward them to our office. However, new conditions and emergency situations often require new radiographs to ensure an accurate diagnosis. If additional images are necessary, they can be taken at our facility using our state-of-the-art radiographic equipment.
Medical & Dental History
To provide the most appropriate care, we need an accurate understanding of your medical and dental background. This includes previous dental treatments and restorations, current or past medical conditions and any existing radiographs.
With your authorization, your previous dental office may forward your records to us. You will also be asked to complete a new Patient History Form at your first visit, so that our records are fully up to date.
Insurance Information
After each appointment, our administrative team will submit your insurance claim forms to your insurance provider on your behalf.
Please be prepared to provide your insurer’s name and mailing address, as well as your group or policy number and your personal identification or certificate number at your first visit.
